About the role

Responsibilities

  • Conduct comprehensive in-home clinical assessments, including medical history reviews, medication reconciliation, and physical exams
  • Manage members through acute medical situations or chronic condition exacerbations, including diagnostics and interventions
  • Manage members during short-term sub-acute rehabilitation episodes
  • Ensure accurate and complete clinical documentation, including ICD-10 coding
  • Collaborate with an interdisciplinary care team to coordinate patient care
  • Lead advanced illness and advanced care planning conversations
  • Provide essential counseling and education to patients and their caregivers

Requirements

  • For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • For PAs: Current NCCPA certification and state licensure
  • Active, unrestricted state licensure in the practicing state (or ability to obtain by start date)
  • Active and unrestricted driver's license and access to reliable transportation for daily patient home visits
  • Current, active DEA licensure/prescriptive authority
  • Ability to navigate varied home environments and perform physical duties such as sitting, standing, or kneeling as needed

Preferred Qualifications

  • 1+ years of clinical experience as an Advanced Practice Clinician (family, geriatric, or home health preferred)
  • Experience in geriatric medicine, long-term care, senior living, or home care settings
  • Experience managing patients with complex behavioral, social, or functional needs
  • Strong understanding of Geriatrics, Chronic Illness, and end-of-life discussions
  • Proficiency with electronic medical records (EMR) and healthcare technology
