About the role
Responsibilities
- Conduct comprehensive clinical assessments, including medical history reviews, medication reconciliation, and physical exams
- Perform evidence-based screenings and point-of-care testing as appropriate
- Support members during short-term sub-acute rehabilitation episodes
- Ensure accurate and complete clinical documentation, including ICD-10 coding
- Collaborate with interdisciplinary care teams to coordinate member health
- Lead advanced illness and advanced care planning conversations
- Provide essential counseling and education to patients and their caregivers
Requirements
- For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- For PAs: Current NCCPA certification and state licensure
- Active, unrestricted state licensure in the practicing state without disciplinary actions
- Active and unrestricted driver's license with access to reliable transportation
- Current, active DEA licensure/prescriptive authority
- Ability to obtain a collaborative practice agreement if required by state regulations
Preferred Qualifications
- 1+ years of clinical experience as an Advanced Practice Clinician (long-term care preferred)
- Experience in geriatric medicine, long-term care, senior living, or home care settings
- Experience managing patients with complex behavioral, social, or functional needs
- Strong understanding of Geriatrics, Chronic Illness, and end-of-life discussions
- Proficiency with electronic medical records (EMR) and healthcare technology
