Nurse Practitioner or Physician Assistant

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients
• Conduct comprehensive medical evaluations
• Develop and manage individualized treatment plans
• Prescribe and monitor medications
• Coordinate with physicians, caregivers, and interdisciplinary team members
• Help patients and families understand diagnoses and manage chronic illness

Requirements

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
• Current State License as a Nurse Practitioner or Physician Assistant
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage
• Ability to travel daily within the service area to patient homes and facilities

Preferred Qualifications

• Minimum 2 years clinical experience in a post-acute setting
• Adult, Family, or Gerontology certification
• Experience with geriatric patients
