About the role
Responsibilities
- Provide primary and palliative care to homebound patients in Mercer County, NJ
- Perform physical examinations and assess patient health histories
- Develop diagnoses and treatment plans for chronic conditions
- Order diagnostic tests and prescribe medications as needed
- Communicate with office staff and clinical operations via internal platforms
- Attend quarterly medical staff meetings
Requirements
- Board Certified Nurse Practitioner or Physician Assistant
- Valid NP or PA certification in New Jersey
- 1-2 years of primary care experience with geriatric patients
- Strong clinical skills in assessment, diagnosis, and treatment
- Hospice experience preferred
