Nurse Practitioner or Physician Assistant

Responsibilities

• Provide primary and palliative care to homebound patients in Passaic and Hudson County, NJ
• Perform physical examinations and assess patient health histories
• Develop impressions, diagnoses, and treatment plans for a panel of patients
• Order diagnostic tests and prescribe medications and supplies as medically necessary
• Communicate effectively with office staff and clinical operations across internal platforms
• Foster trust with patients, families, and caregivers during in-home visits
• Attend quarterly medical staff meetings

Requirements

• Board Certified NP or PA with valid certification in New Jersey
• 1-2 years of primary care experience with geriatric patients
• Strong clinical skills to accurately assess, diagnose, and treat patients
• Confidence to problem-solve and operate independently in the field

Preferred Qualifications

• Hospice or palliative care experience
