Nurse Practitioner or Physician Assistant

Responsibilities

• Provide primary and palliative care to homebound patients in Morris County, NJ
• Perform physical examinations and review medical histories
• Develop diagnoses and treatment plans for chronic conditions
• Order diagnostic tests and prescribe medications and supplies
• Communicate with office staff and clinical operations through internal platforms
• Attend quarterly medical staff meetings
• Build trust with patients, families, and caregivers during visits

Requirements

• Board Certified NP or PA with valid New Jersey certification
• 1-2 years of primary care experience with geriatric patients
• Strong clinical skills to assess, diagnose, and treat accurately
• Ability to work independently and problem-solve
• Positive demeanor and strong interpersonal skills

Preferred Qualifications

• Hospice or palliative care experience
