About the role
Responsibilities
- Provide primary and palliative care to homebound patients in Sussex County, NJ
- Perform physical examinations and assess patient health histories
- Develop diagnoses and treatment plans for chronic conditions
- Order diagnostic tests and prescribe medications and supplies
- Communicate with office staff and attend quarterly medical staff meetings
- Consult with patients, families, and caregivers during home visits
Requirements
- Board Certified Nurse Practitioner or Physician Assistant with valid New Jersey license
- 1-2 years of primary care experience with geriatric patients
- Hospice experience highly preferred
- Strong clinical skills in assessment, diagnosis, and treatment
- Ability to operate independently and problem-solve effectively
