Nurse Practitioner or Physician Assistant

Responsibilities

• Provide primary and palliative care to homebound patients in Sussex County, NJ
• Perform physical examinations and assess patient health histories
• Develop diagnoses and treatment plans for chronic conditions
• Order diagnostic tests and prescribe medications and supplies
• Communicate with office staff and attend quarterly medical staff meetings
• Consult with patients, families, and caregivers during home visits

Requirements

• Board Certified Nurse Practitioner or Physician Assistant with valid New Jersey license
• 1-2 years of primary care experience with geriatric patients
• Hospice experience highly preferred
• Strong clinical skills in assessment, diagnosis, and treatment
• Ability to operate independently and problem-solve effectively
