Nurse Practitioner or Physician Assistant

Responsibilities

• Provide primary and palliative care to homebound patients in Atlantic County, NJ
• Perform physical examinations and assess patient health histories
• Develop diagnoses, treatment plans, and impressions for chronic disease management
• Order diagnostic testing and prescribe medications and supplies as medically necessary
• Communicate effectively with office staff and clinical operations via internal platforms
• Foster trust with patients, families, and caregivers during in-home visits
• Attend quarterly medical staff meetings

Requirements

• Board Certified Nurse Practitioner or Physician Assistant with valid New Jersey certification
• 1-2 years of primary care experience with geriatric patients
• Strong clinical skills to accurately assess, diagnose, and treat patients
• Ability to problem solve and operate independently in the field
• Positive demeanor and strong interpersonal skills
