About the role
Responsibilities
- Conduct visits with hospice patients in the Voorhees, NJ region
- Work independently under the supervision of a physician
- Complete physical exams and review of systems to assess hospice eligibility
- Communicate with patients, families, and caregivers during visits
- Attend IDG meetings when possible
- Prescribe medications, supplies, and medically necessary interventions
Requirements
- Valid certification in the state of job location
- 1-2 years of primary care experience with older adults
- Strong rapport building and interpersonal skills
- Excellent communication and presentation skills
- Demonstrated collaboration skills
