About the role

Responsibilities

  • Conduct visits with hospice patients in the Voorhees, NJ region
  • Work independently under the supervision of a physician
  • Complete physical exams and review of systems to assess hospice eligibility
  • Communicate with patients, families, and caregivers during visits
  • Attend IDG meetings when possible
  • Prescribe medications, supplies, and medically necessary interventions

Requirements

  • Valid certification in the state of job location
  • 1-2 years of primary care experience with older adults
  • Strong rapport building and interpersonal skills
  • Excellent communication and presentation skills
  • Demonstrated collaboration skills
