Nurse Practitioner

Responsibilities

• Conduct comprehensive in-home health assessments including medical history review and physical exams
• Perform medication reconciliation and vital signs monitoring
• Execute evidence-based screenings and point-of-care testing
• Identify and document diagnoses for care management and treatment planning
• Communicate clinical findings to primary care providers to address gaps in care
• Educate members on disease processes, medications, and compliance
• Address social determinants of health and provide necessary referrals
• Recognize and intervene in urgent or emergent situations

Requirements

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (for NPs)
• Current NCCPA certification and state licensure (for PAs)
• Active, unrestricted licensure in the state of practice
• Active and unrestricted driver's license
• Access to reliable transportation for home visits
• Ability to navigate varied home environments and perform physical assessments

Preferred Qualifications

• 1+ years of clinical experience in family, geriatric, or home health
• Proficiency with electronic medical records and technology
• Ability to transport equipment weighing up to 30 pounds
• Experience communicating with geriatric or Medicare populations
