About the role
Responsibilities
- Conduct comprehensive in-home clinical assessments, including medical history reviews, medication reconciliation, and physical exams
- Manage members through acute medical situations or chronic condition exacerbations, including diagnostics and interventions
- Oversee member care during short-term sub-acute rehabilitation episodes
- Ensure accurate and complete clinical documentation, including ICD-10 coding
- Collaborate with an interdisciplinary care team to coordinate patient needs
- Lead advanced illness and advanced care planning conversations
- Provide essential counseling and education to patients and their caregivers
Requirements
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- Active, unrestricted state licensure in the practicing state (or ability to obtain by start date)
- Active and unrestricted driver's license and access to reliable transportation for daily patient home visits
- Current, active DEA licensure/prescriptive authority
- Ability to navigate varied home environments and perform physical duties such as sitting, standing, or kneeling as needed
Preferred Qualifications
- 1+ years of clinical experience as an Advanced Practice Clinician (family, geriatric, or home health preferred)
- Experience in geriatric medicine, long-term care, senior living, or home care settings
- Experience managing patients with complex behavioral, social, or functional needs
- Strong understanding of Geriatrics, Chronic Illness, and end-of-life discussions
- Proficiency with electronic medical records (EMR) and healthcare technology
