About the role

Responsibilities

  • Conduct comprehensive in-home health assessments, including medical history reviews, medication reconciliation, and physical exams
  • Perform evidence-based screenings and point-of-care testing as appropriate
  • Identify and document diagnoses to assist with care management and treatment planning
  • Communicate clinical findings to members' primary care providers to address gaps in care
  • Educate members on disease processes, medications, and treatment compliance
  • Address social determinants of health and provide necessary referrals
  • Recognize and intervene in urgent or emergent situations during home visits

Requirements

  • Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • Active, unrestricted state licensure in the practicing state (or ability to obtain by start date)
  • Active and unrestricted driver's license
  • Access to reliable transportation for conducting home visits
  • Ability to navigate varied home environments and perform physical assessments (sitting, standing, kneeling)

Preferred Qualifications

  • 1+ years of clinical experience, specifically in family, geriatric, or home health settings
  • Proficiency with electronic medical records (EMR) and healthcare technology
  • Ability to transport medical equipment weighing up to 30 pounds and navigate stairs
  • Proven communication skills with geriatric or Medicare populations
