Responsibilities

  • Manage a panel of families as the primary care provider for longitudinal primary, preventive, acute, and pediatric care
  • Deliver care in home settings and via telehealth/virtual modalities (video, phone, SMS)
  • Collaborate with an interdisciplinary team including MDs, LCSWs, RNs, and dieticians
  • Utilize electronic health records to document interactions, track tasks, and communicate with the team
  • Propose treatments for chronic and infectious diseases and prescribe/reconcile medications
  • Order, administer, and record diagnostic tests
  • Provide advocacy, navigation support, and education regarding health and social care services
  • Participate in outreach efforts to explain program details to potential families

Requirements

  • Completion of an accredited Nurse Practitioner program
  • Active, unrestricted license to practice in Arizona
  • At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients
  • Experience with both chronic medical and behavioral health conditions
  • Proficiency in clinical skills such as venipuncture, phlebotomy, and patient assessments
  • Current CPR certification (BLS)
  • Valid driver’s license, reliable transportation, and auto insurance for home visits
  • Ability to lift up to 25 pounds and sit for prolonged periods
  • Availability for a full-time, five-day work week including a rotating call schedule

Preferred Qualifications

  • Experience leading an interdisciplinary care team
  • Experience working in a startup environment
