About the role
Responsibilities
- Manage a panel of families as the primary care provider to improve whole-family health through longitudinal primary and preventive care
- Provide care in home settings and via telehealth and virtual care modalities
- Practice with an interdisciplinary team including MDs, LCSWs, RNs, and dieticians
- Utilize electronic health records to document member interactions and track tasks
- Propose treatments for chronic and infectious diseases and prescribe appropriate medications
- Order, administer, and record diagnostic tests
- Provide advocacy, navigation support, and education to families regarding health and social care services
Requirements
- Completion of an accredited Nurse Practitioner program
- Active, unrestricted license to practice in Arizona
- At least 2 years of experience providing clinical services to pediatric and adult patients
- Experience with both chronic medical and behavioral health conditions
- Proficiency in clinical nursing skills such as venipuncture and phlebotomy
- Current CPR certification (BLS)
- Valid driver's license, reliable transportation, and auto insurance
- Ability to lift up to 25 pounds and travel to family homes
Preferred Qualifications
- Experience leading an interdisciplinary care team
- Experience working in a startup environment
