Responsibilities

• Manage a panel of families as the primary care provider for longitudinal primary, preventive, and pediatric care
• Deliver care in home settings and via telehealth/virtual modalities
• Collaborate with an interdisciplinary team including MDs, LCSWs, RNs, and dieticians
• Utilize electronic health records to document interactions and track tasks
• Propose treatments for chronic and infectious diseases and prescribe medications
• Order, administer, and record diagnostic tests
• Provide advocacy, navigation support, and education regarding health and social services
• Participate in community outreach efforts

Requirements

• Completion of an accredited Physician Assistant or Nurse Practitioner program
• Active, unrestricted license to practice in Louisiana
• At least 2 years of experience providing clinical services to pediatric and adult patients
• Proficiency in clinical nursing skills such as venipuncture and phlebotomy
• Current CPR certification (BLS)
• Valid driver’s license and access to a reliable vehicle for home visits
• Ability to lift up to 25 pounds

Preferred Qualifications

• Experience leading an interdisciplinary care team
• Experience working in a healthcare startup environment
