Nest is a value-based care provider built for families, on a mission to make comprehensive medical, behavioral, and social care radically accessible to America’s highest risk families. Founded in 2022, Nest utilizes a first-of-its-kind family-based, in-home, and virtual care model.

Responsibilities

• Manage a panel of families as the primary care provider to improve whole-family health through longitudinal primary and preventive care
• Provide care in home settings and via telehealth and virtual care modalities
• Practice with the support of an interdisciplinary team including MDs, LCSWs, RNs, and dieticians
• Utilize electronic health records to document member interactions and track tasks
• Propose treatments for chronic and infectious diseases and prescribe appropriate medications
• Order, administer, and record diagnostic tests
• Provide advocacy, navigation support, and education to families regarding health and social care services
• Participate in outreach efforts to explain program details to potential families

Requirements

• Completion of Physician Assistant or Nurse Practitioner accredited program
• Active, unrestricted license to practice in Arizona
• At least 2 years of experience providing clinical services to pediatric and adult patients
• Proficiency in clinical nursing skills such as venipuncture and phlebotomy
• Current CPR certification (BLS)
• Valid driver's license, reliable transportation, and auto insurance
• Ability to lift up to 25 pounds and travel to family homes
• Availability for a full-time, five-day work week including a rotating call schedule

Preferred Qualifications

• Experience working as the leader of an interdisciplinary care team
• Experience in a healthcare startup environment
