About the role
Responsibilities
- Provide medical care, palliation, and management of terminal illness in compliance with the state Nurse Practice Act
- Complete history and physical examinations, including ordering and interpreting diagnostic tests
- Develop individualized plans of care and prescribe medications or treatments
- Complete hospice face-to-face visit requirements to assess eligibility for recertification
- Collaborate with the interdisciplinary team to review and revise patient care plans
- Educate and counsel patients, families, and caregivers regarding medical, psychological, and spiritual needs
- Maintain accurate clinical documentation, including medical orders and assessment notes
Requirements
- Graduation from an accredited School of Nursing and Nurse Practitioner Program
- Master's degree with at least one year of Nurse Practitioner experience
- Minimum of one year of hospice or palliative care experience
- Current nursing licensure in the state and current DEA license
- Current CPR certification
- Valid driver's license and reliable, insured automobile
- Ability to lift up to 50 pounds and perform physical tasks like bending, kneeling, and standing
Preferred Qualifications
- Certification in a specialist area such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics
