Nurse Practitioner

Responsibilities

• Provide medical care, palliation, and management of terminal illness in compliance with the state Nurse Practice Act
• Complete history and physical examinations, including ordering and interpreting diagnostic tests
• Develop individualized plans of care and prescribe medications or treatments
• Complete hospice face-to-face visit requirements to assess eligibility for recertification
• Collaborate with the interdisciplinary team to review and revise patient care plans
• Educate and counsel patients, families, and caregivers regarding medical, psychological, and spiritual needs
• Maintain accurate clinical documentation, including medical orders and assessment notes

Requirements

• Graduation from an accredited School of Nursing and Nurse Practitioner Program
• Master's degree with at least one year of Nurse Practitioner experience
• Minimum of one year of hospice or palliative care experience
• Current nursing licensure in the state and current DEA license
• Current CPR certification
• Valid driver's license and reliable, insured automobile
• Ability to lift up to 50 pounds and perform physical tasks like bending, kneeling, and standing

Preferred Qualifications

• Certification in a specialist area such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics
