Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.

Responsibilities

  • Provide client-centered primary care in a non-judgmental manner to individuals experiencing homelessness and substance use challenges
  • Perform clinical assessments and health information gathering for clients with complex needs
  • Provide nursing services addressing biopsychosocial needs, including comorbid health conditions and mental health diagnoses
  • Complete timely and accurate medical records using the Electronic Medical Record (EMR) system
  • Participate in inter-professional models of care and case conferences
  • Contribute to the development and implementation of medical directives, policies, and protocols
  • Support client safety through adherence to infection prevention and control and safe medication practices

Requirements

  • Master's Degree in Nursing (MSN) with completion of a recognized primary care NP program (NP-PHC)
  • Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, Primary care
  • Minimum of three years’ experience working as a primary care NP with clients having complex needs preferred
  • Experience providing care to individuals facing homelessness or substance use challenges
  • Valid Ontario Driver's license and access to a vehicle
  • Basic CPR certification
  • Ability to work a flexible schedule including at least one evening per week
  • Evidence of valid current vulnerable sector police check

Preferred Qualifications

  • Member of the Nurse Practitioners Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO)
  • Prescribing Narcotics and Controlled Substances certificate endorsed by the CNO
  • Experience working in a CHC setting or community health nursing practice
  • Knowledge of relevant services and community supports in Durham Region
