About the role

Responsibilities

  • Provide client-centered primary care and addiction-focused health services to individuals experiencing homelessness or substance use challenges
  • Perform clinical assessments, diagnosis, and treatment for clients with complex physical, mental health, and substance use concerns
  • Collaborate within an interprofessional model of care to ensure integrated, wraparound supports
  • Complete timely and accurate medical records using the Electronic Medical Record (EMR) system
  • Contribute to the development and implementation of medical directives, policies, and protocols
  • Participate in case conferences, chart audits, and Quality Improvement Plans (QIP)
  • Support client safety through adherence to infection prevention and control and safe medication practices

Requirements

  • Master's Degree in Nursing (MSN) with completion of a recognized primary care NP program (NP-PHC)
  • Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, Primary care
  • Minimum of three years of experience working as a primary care NP with clients having complex needs (preferred)
  • Experience providing care to individuals facing homelessness or substance use challenges
  • Valid Ontario Driver's license and access to a vehicle
  • Basic CPR certification
  • Ability to work a flexible schedule, including at least one evening per week
  • Evidence of a valid current vulnerable sector police check

Preferred Qualifications

  • Membership in the Nurse Practitioners Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO)
  • Prescribing Narcotics and Controlled Substances certificate endorsed by the CNO
  • Experience working in a Community Health Centre (CHC) setting
  • Knowledge of community supports in the Durham Region
