About the role

Responsibilities

  • Deliver youth-centered primary care, including assessments, diagnosis, and treatment
  • Prescribe and manage pharmacotherapy for mental health and medical needs
  • Provide care for biopsychosocial needs, including mental health and substance use
  • Offer guidance on sexual health, sleep, nutrition, and general wellness
  • Conduct intake assessments and support measurement-based care tools
  • Coordinate care through warm handoffs to internal and external partners
  • Participate in program evaluation, clinical audits, and quality improvement
  • Create a safe, non-judgmental, and culturally responsive environment for youth

Requirements

  • Master’s Degree in Nursing (MSN) with NP-PHC completion
  • Current registration with the College of Nurses of Ontario (CNO) as a Nurse Practitioner (Primary Care)
  • Minimum 3 years of experience with complex health, mental health, or substance use needs
  • Lived or living experience with barriers faced by youth
  • Valid Ontario driver’s license and access to a vehicle
  • Basic CPR certification
  • Proficiency with EMRs and Microsoft Office

Preferred Qualifications

  • Experience providing care to youth and marginalized populations
  • Prescribing Narcotics and Controlled Substances certificate endorsed by CNO
  • Knowledge of community health services in Durham Region
  • Experience in community health or integrated care settings
  • Lived/living experience with mental health, substance use, or marginalized populations
