Nurse Practitioner

Responsibilities

• Conduct remote assessments via the Dialogue Care Platform to evaluate symptoms, diagnose, and provide treatment for common and complex conditions
• Deliver high-quality patient care by offering timely medical advice and identifying urgent symptoms
• Create personalized care plans tailored to individual patient needs
• Collaborate closely with an interdisciplinary team of doctors, nurses, psychologists, and care managers to provide comprehensive care
• Educate and support patients on preventative care, health management, and lifestyle changes
• Refer patients to specialists, allied healthcare professionals, or community services when necessary
• Maintain detailed, regulatory-compliant records of patient interactions, treatment plans, and follow-ups

Requirements

• Valid and active medical practice license in Quebec
• Master's degree in Nursing (advanced practice option) and a complementary diploma in primary care
• Proficiency in consulting in either French or English
• Availability to work evenings and weekends for a minimum of 8 hours per week
• Experience with virtual care or high comfort level with digital computer tools
• Ability to work in a fully remote capacity from Quebec

• Experience supporting Women's Health programs
