Responsibilities

• Provide primary care to health center patients using a holistic model focusing on health promotion and behavior change
• Elicit, record, and interpret medical, family, and social history
• Order and perform diagnostic tests, procedures, and therapeutic interventions
• Collaborate with nursing staff to monitor and evaluate patient care quality
• Complete timely documentation in the electronic health record
• Initiate and coordinate referrals to other healthcare team members or agencies
• Incorporate best practices and standards in the care and treatment of all patients
• Maintain effective working relationships with the supervising physician and healthcare team

Requirements

• Master's degree required
• Current license with the New York State Board of Health
• Certified Nurse Practitioner
• Minimum of one year of experience as an NP
• Experience with electronic health record systems
• Knowledge of ICD-9 coding
• Ability to maintain HIPAA standards

Preferred Qualifications

• Master's degree in Nursing
• National Certification
