We focus on Physical Medicine and Rehabilitation (PM&R) to enhance and restore functional ability and quality of life for patients with physical impairments.

Responsibilities

• Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
• Represent CRC in your facility through clinical leadership and collaboration with interdisciplinary care teams
• Collaborate with PCP teams to manage conditions common in SNFs such as CVA, neurological conditions, and postoperative orthopedic surgeries
• Work alongside PT, OT, and SLP teams to improve patient function
• Assess and collaborate on the management of barriers to therapy including pain, spasticity, and neurogenic bladder/bowel
• Attend multidisciplinary meetings to bridge the gap between therapy and internal medicine

Requirements

• Nurse Practitioner/APRN License
• Pennsylvania License
• Ability to work in the U.S.
• Passion for post-acute healthcare and the geriatric population
• Confidence to function autonomously

Preferred Qualifications

• SNF or post-acute experience
