Nurse Practitioner

Responsibilities

• Perform direct patient physical assessments, education, and health planning
• Maintain continuity of care through regular patient follow-up
• Coordinate services with other disciplines and patient insurance plans
• Advocate for patients in arranging medically necessary services and community resources
• Refer patients to specialty services when necessary
• Consult with collaborating physicians and healthcare team members to establish plans of care
• Provide comprehensive case management with an emphasis on health promotion and disease prevention
• Monitor patient outcomes in accordance with practice protocols
• Collaborate with staff nurses and nursing administrators on clinical decision-making
• Act as a resource person and provide in-service education for nursing staff

Requirements

• Graduate of an approved Nurse Practitioner Program
• Current New York State Nurse Practitioner license
• Basic CPR (BCLS) certification
• At least 3 years of clinical experience as a primary care Nurse Practitioner
• Bilingual proficiency in English and Mandarin Chinese
