About the role
Responsibilities
- Perform direct patient physical assessments, education, and health planning
- Maintain continuity of care through regular patient follow-up
- Coordinate services with other disciplines and patient insurance plans
- Advocate for patients in arranging medically necessary services and community resources
- Refer patients to specialty services when necessary
- Consult with collaborating physicians and healthcare team members to establish plans of care
- Provide comprehensive case management with an emphasis on health promotion and disease prevention
- Monitor patient outcomes in accordance with practice protocols
- Collaborate with staff nurses and nursing administrators on clinical decision-making
- Act as a resource person and provide in-service education for nursing staff
Requirements
- Graduate of an approved Nurse Practitioner Program
- Current New York State Nurse Practitioner license
- Basic CPR (BCLS) certification
- At least 3 years of clinical experience as a primary care Nurse Practitioner
- Bilingual proficiency in English and Mandarin Chinese

- Annual base salary of $105,000 - $115,000 plus productivity-based incentive pay
