About the role
Responsibilities
- Provide primary medical management in collaboration with PACE Organization Physicians
- Conduct patient assessments and develop comprehensive care plans
- Participate in discharge planning and provide treatment for acute illnesses
- Conduct home visits to participants when necessary
- Assist in the formulation of clinical policies and procedures
- Participate in the provider on-call rotation
Requirements
- Master's degree in Nursing
- 2-5 years of clinical experience
