Nurse Practitioner

Responsibilities

• Perform detailed physical, functional, and psychosocial assessments for new participants
• Order and interpret diagnostic tests
• Develop and update individualized treatment plans, including establishing goals of care and prescribing medications
• Provide education to participants, caregivers, and staff regarding health conditions and treatments
• Participate in interdisciplinary team meetings and family conferences
• Ensure accurate and timely documentation of all assessments and interventions
• Manage acute and chronic conditions in collaboration with other providers

Requirements

• Current CA State issued Nurse Practitioner license
• Current active DEA license
• 1+ year as a practicing Nurse Practitioner
• 5+ years as a Registered Nurse
• Experience in elderly care or geriatric settings
