About the role

Responsibilities

  • Perform and document comprehensive medical, surgical, and psychosocial health histories and physical examinations.
  • Diagnose and manage acute and chronic conditions in adult patients and conduct routine wellness visits.
  • Order and interpret diagnostic tests, prescribe treatments, and administer medications and immunizations.
  • Develop goal-directed care plans based on evidence-based guidelines and institutional protocols.
  • Counsel and educate patients and caregivers on disease prevention, lifestyle modifications, and care plans.
  • Coordinate transitions of care, including discharge assessments and follow-up coordination.
  • Collaborate with a multidisciplinary team including physicians, specialists, social workers, and pharmacy.
  • Document all patient interactions accurately within the outpatient electronic medical record (EMR).
  • Participate in clinical team meetings and contribute to standardized workflows and training initiatives.

Requirements

  • Master’s degree in Nursing from an accredited program or equivalent education and experience.
  • 1-2 years of related clinical experience.
  • Current New York State Nurse Practitioner and Registered Professional Nurse licenses.
  • NP Board Certification (AGACNP, AGPCNP, or FNP).
  • Eligibility for NPI registration and DEA certification.
  • Eligibility for American Heart Association (AHA) ACLS and BLS.
  • Proficiency with electronic medical records (EMR) and Microsoft Office Suite.

Preferred Qualifications

  • At least three years of experience managing adult patients in a primary care setting.
  • At least two years of prior clinical experience in a hospital or relevant clinical setting.
