Nurse Practitioner

Responsibilities

• Provide frontline care to patients experiencing homelessness, behavioral health challenges, serious mental illness, and substance use disorders
• Manage diverse patient panels by delivering acute, chronic, and preventative care in the field
• Conduct patient assessments, physical exams, and order/interpret diagnostics
• Design and implement care plans to support patient stability and health outcomes
• Collaborate with a multidisciplinary team including physicians, nurses, and social workers
• Document all encounters to maintain accurate medical records

Requirements

• 2+ years of experience working with CalAIM populations of focus
• Experience in street medicine or related outreach settings
• Active, unrestricted California Registered Nurse (RN) license
• Active California Nurse Practitioner certification
• National Nurse Practitioner board certification (AANP or ANCC)
• California Furnishing Number and DEA registration
• National Provider Identifier (NPI) number
• Basic Life Support (BLS) certification
• Valid CA Driver’s License and reliable, insured vehicle for up to 75% travel

Preferred Qualifications

• Experience working with diverse and underserved populations
• Familiarity with value-based care models
• Bilingual abilities (Spanish or other languages)
• Local Bay Area residency
