Responsibilities

• Travel to members’ homes to conduct comprehensive health evaluations and Annual Wellness Visits
• Perform medical history, physical examinations, vital signs collection, and medication reconciliation
• Conduct preventive health screenings including Hemoglobin A1C, Spirometry, KED, DRE, FIT, and Bone Density
• Deliver preventive health education and counseling on chronic condition management
• Identify clinically relevant diagnoses for care management and risk adjustment documentation
• Communicate findings to primary care providers via post-visit summaries
• Identify and help close quality care gaps such as screenings, labs, and follow-ups
• Maintain accurate and compliant EMR documentation in adherence to HIPAA regulations

Requirements

• Active, unencumbered Nurse Practitioner (NP) license in Oregon
• Minimum 3 years of direct patient care experience (primary care, adult/geriatric preferred)
• Proficiency with EMR systems
• Ability to travel within a 55-mile radius of assigned home location and statewide with notice

Preferred Qualifications

• Prior in-home risk assessment experience
• Bilingual proficiency
