Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments
• Perform focused histories, physical exams, vitals, medication reviews, and screenings
• Perform specific screenings including Hemoglobin A1C, Spirometry, KED, DRE, and FIT
• Educate members on preventive care and chronic disease management
• Document all visits accurately in the EMR to support value-based care and risk adjustment

Requirements

• Active, unencumbered Nurse Practitioner (NP) license in Mississippi
• Ability to maintain a minimum commitment of approximately 30 hours per month
• Willingness to travel daily within a 55-mile radius of the assigned county
• Ability to work independently and professionally in a field setting
• Proficiency with EMR systems

Preferred Qualifications

• 3+ years of patient care experience
• Previous experience with in-home assessments or risk adjustment
