Nurse Practitioner

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments
• Perform focused histories, exams, vitals, medication reviews, and screenings
• Perform screenings including Hemoglobin A1C, Spirometry, KED, DRE, and FIT
• Educate members on preventive care and chronic disease management
• Document visits in the EMR to support value-based care and risk adjustment

Requirements

• Active, unencumbered NP license in the applicable state
• Reliable transportation for travel within a 55-mile radius
• Ability to work independently and maintain ongoing monthly availability
• Proficiency with EMR systems
• Minimum commitment of approximately 30 hours per month

Preferred Qualifications

• 3+ years of patient care experience (primary care, adult, or geriatric)
• Previous in-home or risk-adjustment experience
• Previous 1099 (PRN) experience
• Bilingual skills
