About the role
Responsibilities
- Conduct in-home Annual Wellness Visits and chronic condition assessments
- Perform focused histories, exams, vitals, medication reviews, and screenings
- Perform screenings including Hemoglobin A1C, Spirometry, KED, DRE, and FIT
- Educate members on preventive care and chronic disease management
- Document visits in the EMR to support value-based care and risk adjustment
Requirements
- Active, unencumbered NP license in the applicable state
- Reliable transportation for travel within a 55-mile radius
- Ability to work independently and maintain ongoing monthly availability
- Proficiency with EMR systems
- Minimum commitment of approximately 30 hours per month
Preferred Qualifications
- 3+ years of patient care experience (primary care, adult, or geriatric)
- Previous in-home or risk-adjustment experience
- Previous 1099 (PRN) experience
- Bilingual skills
