About the role
Responsibilities
- Coordinate care for patients within the Neurosciences service line, acting as a clinical liaison across medical services and clinical departments.
- Provide patient education regarding diseases, treatments, side effects, and adverse reactions based on multidisciplinary care plans.
- Facilitate resolution to patient care gaps by proactively reaching out to patients and addressing direct inquiries.
- Assess clinical, emotional, spiritual, psychosocial, and financial patient needs.
- Evaluate and manage patient symptoms before, during, and after treatment.
- Review new patient intake information to determine appropriate scheduling pathways.
- Develop or coordinate the development of patient education materials and clinical pathways.
Requirements
- Registered Nurse (RN) with a current Illinois license.
- Minimum of two years of clinical nursing experience with demonstrated competence in patient care, teaching, and management.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and physicians.
- General computer skills and adaptability to changing work assignments.
Preferred Qualifications
- Certified Nurse in a specialty area (or currently in the process of obtaining certification).
