Nurse Navigator, Post-Heart Transplant Program

Responsibilities

• Coordinate care and provide patient education for individuals in the post-heart transplant service line
• Act as a clinical liaison between patients, families, and multidisciplinary clinical teams
• Support discharge planning and facilitate the resolution of patient care gaps
• Monitor follow-up and proactively address patient questions to prevent readmissions or ER visits
• Educate patients and families on diseases, treatments, side effects, and reportable symptoms
• Review clinical documentation to improve length of stay (LOS)
• Assist in the development of patient education materials, clinical pathways, and quality studies

Requirements

• Actively licensed Registered Nurse (RN) in the state of Illinois
• Minimum of two years of clinical nursing experience
• Demonstrated clinical competence in patient care, teaching, and patient management
• General computer skills and ability to adapt to changing work assignments
• Excellent interpersonal and communication skills for interacting with patients, families, and physicians

Preferred Qualifications

• Certified Nurse in a specialty area or currently in the process of obtaining certification
