About the role
Responsibilities
- Coordinate care and provide patient education for individuals in the post-heart transplant service line
- Act as a clinical liaison between patients, families, and multidisciplinary clinical teams
- Support discharge planning and facilitate the resolution of patient care gaps
- Monitor follow-up and proactively address patient questions to prevent readmissions or ER visits
- Educate patients and families on diseases, treatments, side effects, and reportable symptoms
- Review clinical documentation to improve length of stay (LOS)
- Assist in the development of patient education materials, clinical pathways, and quality studies
Requirements
- Actively licensed Registered Nurse (RN) in the state of Illinois
- Minimum of two years of clinical nursing experience
- Demonstrated clinical competence in patient care, teaching, and patient management
- General computer skills and ability to adapt to changing work assignments
- Excellent interpersonal and communication skills for interacting with patients, families, and physicians
Preferred Qualifications
- Certified Nurse in a specialty area or currently in the process of obtaining certification
