Nurse Manager - Homeless Services

Responsibilities

• Supervise nurses, nursing students, and case management staff
• Manage a multidisciplinary team to link clients with serious mental illness, substance use disorders, and complex medical needs to primary care, mental health care, and substance use treatment
• Assist clients with medication management, referrals, specialized case management, and systems advocacy
• Support chronic disease management and medication adherence through education, pill boxes, refill management, and reminders
• Establish collaborative relationships with outside agencies and make appropriate referrals
• Meet with clients on the street, in treatment programs, hospitals, and correctional facilities to maintain continuity of care
• Provide harm reduction counseling and refer clients to safer drug use supplies, monitoring, or medication-assisted treatment
• Provide consultation and guidance to Homeless Services Division leadership and teams
• Participate in strategic planning and leadership forums within the division

Requirements

• Licensed Registered Nurse with a minimum of a bachelor's degree in nursing
• At least 4 years' experience working with adults with complex medical and/or behavioral health diagnoses
• Working knowledge of psychiatric medications and psychiatric and medical treatments
• Experience working with patients experiencing homelessness with severe trauma, mental health diagnoses, and substance use disorders
• Demonstrated ability to engage patients actively using substances and those in all phases of recovery
• Strong interpersonal skills and ability to collaborate with diverse outreach workers, recovery coaches, and medical providers
• Ability to work autonomously in clinical and street-based settings
• Demonstrated ability to provide care using harm reduction principles and overdose prevention/response
• Valid driver's license and reliable transportation for work and client transport
