Responsibilities

• Perform medical assessments
• Work in a team with other healthcare professionals to meet patient needs
• Document in journal systems including Take Care, Appva, and Treserva
• Use Mobil Klinik for care planning
• Ensure patients and their relatives feel secure and involved in their care
• Provide care and support to individuals with special needs in their homes

Requirements

• Educated nurse (sjuksköterska)
• Ability to work independently and make autonomous assessments
• Valid driver's license
• Positive and proactive attitude
• Strong collaborative skills
• Responsible, flexible, and independent in professional practice

Preferred Qualifications

• Previous experience in on-call work within LSS and Säbo
• Experience in advanced home healthcare
