Nurse Case Manager

Responsibilities

• Act as a primary case manager to 3-6 consumers
• Provide nursing interventions to individuals
• Facilitate and coordinate medical care including communication with health care providers
• Provide health education to individuals
• Assist in the management of medications and the overall functioning of the nursing department
• Formulate and regularly update Comprehensive Assessments and IAPs for clients on your caseload
• Transport consumers to work sites, meetings, daily activities, and appointments as needed
• Participate in rotating on-call coverage and evening shifts to ensure 24/7 support

Requirements

• Associate degree in nursing
• LPN or RN nursing license
• Experience working with adults with mental illness
• Knowledge of psychiatric and medical medications
• Valid Massachusetts driver's license
