Responsibilities

• Provide psychosocial services and counseling to in-center and home dialysis patients to promote positive adjustment and improved quality of life
• Conduct patient assessments to identify psychosocial strengths, needs, and barriers to treatment
• Collaborate with the interdisciplinary healthcare team to develop and implement effective care plans
• Provide supportive and goal-directed counseling, including assistance for patients seeking kidney transplants
• Facilitate referrals to community resources such as vocational rehabilitation and home health services
• Educate patients and families on kidney disease, treatment modalities, and psychosocial impacts of End Stage Renal Disease (ESRD)
• Assist patients with navigating insurance, financial assistance, and advance directive discussions
• Supervise and monitor the clinical work of supervisees to ensure compliance with laws and high-quality care standards
• Participate in quality review meetings to monitor adherence and patient satisfaction indicators

Requirements

• Master's degree in Social Work (MSW)
• Valid state-specific Social Work licensure
• 2–5 years of related professional experience
