MSW - Hospice Social Worker

Responsibilities

• Provide medical social services under the direction of a physician and Interdisciplinary Group
• Assist patients, families, and healthcare teams in understanding social, emotional, and environmental factors related to health status
• Contribute to the development of a comprehensive, integrated Plan of Care on a daily basis
• Instruct healthcare team members on available community resources to assist patients

Requirements

• Master's Degree in Social Work from a CSWE accredited school
• Licensed Social Worker (LSW) from the State Board of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle

Preferred Qualifications

• 1+ years of social work experience in a healthcare setting
• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence
