About the role
Responsibilities
- Coordinate direct case management services for incarcerated individuals receiving Medication for Opioid Use Disorder (MOUD) or those on waitlists.
- Develop collaborative, individualized release plans to facilitate successful community reentry.
- Connect individuals to community-based resources including substance abuse treatment, mental health services, housing, and peer support.
- Triage case management services based on vulnerability factors such as homelessness, age, gender, and medical needs.
- Collaborate with corrections officers, jail medical/mental health staff, and community partner organizations.
- Maintain required written documentation and perform program analysis and research.
Requirements
- Master's degree in social work, counseling, mental health, behavioral sciences, or a related field plus 1 year of experience working with persons with legal system involvement; OR
- Bachelor's degree in social work, counseling, mental health, behavioral sciences, or a related field plus 2 years of experience working with persons with legal system involvement; OR
- 5 years of equivalent professional experience combining training and related knowledge.
- Knowledge of case management, reentry practices, and trauma-informed care.
- Ability to work effectively in a secured jail environment.
Preferred Qualifications
- Experience working with individuals with substance use disorders, specifically Opioid Use Disorder and MOUD.
- Experience managing complex co-occurring disorders in adults.
- Previous experience working within correctional facilities or with incarcerated populations.
- Strong background in professional case management.
