About the role
Responsibilities
- Provide person-centred, trauma-informed mental health support to promote independence and skill development
- Increase individuals' community living skills, personal growth, and social engagement
- Build rapport with clients to understand their needs and help implement effective solutions
- Prompt and encourage individuals regarding activities of daily living, including personal care and meal preparation
- Support individuals in developing goals and support plans in partnership with stakeholders
- Monitor daily behaviours and provide feedback to management regarding the effectiveness of support plans
- Maintain high-quality progress notes and documentation in line with NDIS and funding body requirements
Requirements
- Certificate IV in Mental Health or a similar qualification
- Demonstrated experience working with people with complex needs and psychosocial disabilities
- Current driver's license
- Current First Aid & CPR certificate
- Ability to work Tuesday to Friday, 7am to 4pm (72 hours per fortnight)
- Ability to clear probity checks, including a National Criminal History Record Check and WA Working with Children Check
Preferred Qualifications
- Previous experience with disability support, specifically prompting for personal care and meal preparation
