About the role

Responsibilities

  • Support program alumni in managing crises and connecting with social networks
  • Assist individuals with mental health needs in their homes and communities
  • Maintain accurate daily logs and documentation in the electronic health record (EHR) system
  • Coordinate with state and local agencies to ensure effective delivery of services
  • Provide timely communication to the Assistant Director regarding individual progress and concerns
  • Attend all necessary meetings related to support activities and professional supervision
  • Complete personal care tasks as needed to ensure high quality of care

Requirements

  • High school diploma or GED required
  • One year of experience in direct care or working with individuals with mental health disorders
  • Valid U.S. driver's license held for at least two years
  • Access to a reliable vehicle for travel during work hours
  • Ability to pass background checks, physical examinations, and clearances

Preferred Qualifications

  • Bachelor’s degree in a related human services field (e.g., psychology, social work, or special education)
  • Certification from a County CPS program
