About the role
Responsibilities
- Conduct in-person outreach and enrollment conversations with eligible community members to drive same-day registrations.
- Guide individuals through the registration, intake, and enrollment processes to maximize conversion.
- Execute enrollment activities at assigned partner sites, community locations, and Zócalo-hosted events.
- Maintain disciplined follow-up to move referrals and leads into enrolled members.
- Accurately complete registrations and maintain timely activity tracking within Athena and other operational systems.
- Build trust quickly and effectively with community members to facilitate care connection.
Requirements
- 1–3+ years of experience in community outreach, healthcare enrollment, public health, sales, or field-based roles.
- Bilingual English-Spanish fluency is required.
- Reliable transportation and willingness to travel locally within the field.
- Ability to work a flexible schedule, including evenings and weekends as needed.
- Strong communication, relationship-building, and organizational skills.
- Comfort initiating conversations with strangers in public settings and working toward performance metrics.
Preferred Qualifications
- Experience with Medi-Cal, ECM, Community Supports, or community-based healthcare programs.
- Experience using Athena health or comparable electronic health record systems.
- Familiarity with local community resources and the populations served in Santa Clara County.
