About the role
Responsibilities
- Conduct thorough psychosocial screenings, assessments, and evaluations for transplant patients and living donors to establish individualized care plans.
- Manage psychosocial needs through all phases of transplantation, including diagnosis, treatment, and follow-up.
- Collaborate with a multidisciplinary team of physicians, transplant coordinators, and community agencies to optimize patient outcomes.
- Facilitate effective communication between administration, medical staff, patients, and families.
- Document psychosocial assessments accurately within the Electronic Medical Record (EMR) to ensure regulatory compliance.
- Act as an advocate and liaison for patients, participating in selection committees, multidisciplinary rounds, and discharge planning.
- Identify and assess psychosocial and financial problems, providing referrals to appropriate community resources.
- Maintain up-to-date knowledge of CMS, UNOS/OPTN regulatory requirements, and third-party payer requirements.
Requirements
- Master's degree in Social Work.
- Licensed Master Social Worker (LMSW) credential.
- Current New Mexico state licensure (LISW, LCSW, or LMSW).
- 5 years of hospital social work or discharge planning experience in a relevant setting.

- Direct experience in transplant services or inpatient hospital social work.
