Medical Social Worker

Responsibilities

• Assess the psychosocial status of patients and families/caregivers related to terminal illness
• Conduct social evaluations including family dynamics, caregiver abilities, and risks for abuse or neglect
• Provide counseling and crisis intervention for patients and families regarding stress and coping
• Educate interdisciplinary groups on cultural needs, traditions, and taboos
• Maintain clinical records and assist in the preparation of advanced directives
• Provide information and referral services for practical and environmental needs
• Serve as a liaison between patients, families, and community agencies
• Participate in individualized plans of care and interdisciplinary group meetings
• Assist with funeral arrangements and financial or legal decision-making planning

Requirements

• Master's degree in Social Work (MSW) from a CSWE-accredited program OR a Bachelor's degree in Social Work from a CSWE-accredited program
• Minimum of one year of experience in healthcare
• Current CPR Certification
• Valid driver's license and reliable, insured vehicle
• Ability to lift and/or move up to 50 pounds

Preferred Qualifications

• Previous experience in hospice care
• Deep understanding of hospice philosophy and end-of-life issues
