Requirements
Degree or postgraduate diploma in Social Work, 2 years relevant experience preferred, Interest in working with elderly, Strong communication and interpersonal skills
About the role
Responsibilities
- Explore and provide psycho-social and emotional care to patients and caregivers
- Lead Advance Care Planning (ACP) discussions and complete Preferred Care Plans
- Provide bereavement support and crisis intervention
- Coordinate in-house financial assistance for needy patients
- Coordinate medical escort, transport, respite care, and nursing home referrals
- Provide counseling and reconciliation services
- Engage in education, teaching, and networking
Requirements
- Degree or postgraduate diploma in Social Work
- Preferably 2 years of relevant experience
- Interest in working with the elderly
- Compassionate, positive, and caring attitude
- Excellent communication, interpersonal, and problem-solving skills
- Meticulous, diligent, and a strong team player
- Adaptable to changes
