Medical Social Worker

Responsibilities

• Manage referral process, intake screening, admission, financial counselling, and orientation for new dialysis patients
• Conduct needs assessments and formulate tailored care plans for ESRD patients and their families
• Monitor patient well-being during dialysis sessions and maintain therapeutic rapport
• Identify and flag patients at risk of psychosocial deterioration, treatment non-adherence, or financial hardship for early intervention
• Conduct home or hospital visits for holistic care planning when necessary
• Plan, design, and implement patient-centred programmes and psychoeducation sessions focused on quality of life and social integration
• Serve as primary contact for befriender volunteers, providing training, supervision, and ongoing support
• Maintain timely and accurate case records and prepare periodic reports for management review

Requirements

• Degree in Social Work or equivalent recognised by SASW
• Relevant experience in medical or community social work preferred
• Strong interpersonal, communication, and organisational skills
• Ability to work collaboratively in a multidisciplinary setting
