About the role
Responsibilities
- Create and maintain separate patient medical records using authorized recordkeeping processes
- Maintain patient records in an electronic health record system ensuring compliance with interoperability and reporting standards
- Process medical record requests from authorized health services and government representatives in accordance with privacy laws
- Protect patient confidentiality and PHI in compliance with HIPAA and other federal/state requirements
- Support timely retrieval, release, and secure transmission of medical information for continuity of care and audits
- Prepare and transfer confidential medical records or summaries for patient movement between facilities
- Maintain accuracy and organization of medical documentation and escalate discrepancies
- Support daily tracking of medical requests and appointments within facility information systems
- Maintain records in accordance with National Archives and Records Administration schedules
- Participate in required training related to privacy, records management, and emergency response
Requirements
- Experience maintaining medical or health records in a healthcare setting
- Experience working with an electronic health record system
- Knowledge of medical record confidentiality, retention, and release-of-information requirements
- Ability to protect PHI and sensitive information
- Proficiency with scanning equipment, printers, and computers
- Basic Life Support (BLS) certification
- High School Diploma or GED equivalent
- U.S. citizenship and five-year U.S. residency
- Ability to pass a medical examination and background investigation
Preferred Qualifications
- Verbal and written proficiency in Spanish
- Experience in a detention, correctional, or residential healthcare setting
