Medical Home Direct Support Professional

Responsibilities

• Assist individuals with planned activities, community involvement, and social inclusion opportunities.
• Provide medical support including assistance with G-Tubes, catheters, medication administration, and oxygen use.
• Assess pain and discomfort in non-verbal individuals.
• Help individuals develop personal networks and build self-esteem through community participation.
• Provide individualized personal care, including assistance with bathing, showering, and dental hygiene.
• Complete required documentation, including scheduled data collection and monthly progress notes.
• Report any safety, health, or human rights concerns to the Supervisor immediately.

Requirements

• High School Diploma or GED.
• Reliable transportation and access to a personal vehicle during scheduled hours.
• Valid driver's license and proof of automobile insurance.
• Ability to lift up to 50 lbs and perform physical transfers as necessary.
• Ability to work independently and demonstrate sound judgment.

Preferred Qualifications

• Experience in the human services field or working with individuals with developmental disabilities.
