About the role
Responsibilities
- Assist individuals with planned activities and community involvement to promote social inclusion
- Support medical needs including G-tubes, catheters, medication administration, oxygen use, and aspiration precautions
- Assess pain and discomfort in non-verbal individuals
- Provide individualized supports including personal care, hygiene, and skill development
- Develop relationship maps to identify social inclusion opportunities and help build social networks
- Complete required documentation, data collection, and monthly progress notes
- Ensure compliance with state regulations and report all safety or health concerns immediately
Requirements
- High School Diploma or GED
- Reliable transportation
- Valid driver's license
- Proof of automobile insurance
Preferred Qualifications
- Experience in human services or working with individuals with disabilities
