Medical Home Direct Support Professional

Responsibilities

• Assist individuals with planned activities and community involvement to promote social inclusion
• Support medical needs including G-tubes, catheters, medication administration, oxygen use, and aspiration precautions
• Assess pain and discomfort in non-verbal individuals
• Provide individualized supports including personal care, hygiene, and skill development
• Develop relationship maps to identify social inclusion opportunities and help build social networks
• Complete required documentation, data collection, and monthly progress notes
• Ensure compliance with state regulations and report all safety or health concerns immediately

Requirements

• High School Diploma or GED
• Reliable transportation
• Valid driver's license
• Proof of automobile insurance

Preferred Qualifications

• Experience in human services or working with individuals with disabilities
