About the role
Responsibilities
- Serve as attending physician for patients in assigned skilled nursing facilities
- Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision
- Diagnose and treat acute and chronic conditions with patient-centered primary care
- Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and quality initiatives
Requirements
- MD or DO with active U.S. medical license
- Board-certified or board-eligible in an appropriate specialty
- Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, PM&R, or Geriatric Medicine
